The Health Care Program for Children with Special Needs
What are the services of HCP:
HCP offers assistance for families with special needs children (0 to 20 years old) in order to find health care and other resources. Examples might include: seizure disorders, heart conditions, downs syndrome and attention deficit disorder (ADD).

Colorado Women's Cancer Control Initiative
Purpose
To provide screening to eligible women for cancers of the breast and cervix.
Services
Payment and referral for breast exam, pelvic exam, Pap smear and mammogram.
Eligibility
Women who are residents of Rio Blanco County and surrounding area, age 40 and older, within income level guidelines, uninsured or underinsured, have health insurance with high deductibles and/or who are covered by Medicare but have not a had mammogram or pap test in the past year.
Fee
No charge for eligible women ages 40 - 64.

Family Planning Clinic
Purpose
- To improve the reproductive health status of Coloradans by enabling families to achieve their desired fertility;
- to assure access to comprehensive women's health services; and
- to assure access to early and appropriate prenatal services.
Services
- Physical exams including breast and cervical cancer screening.
- Physical examinations including pap smears.
- HIV and other sexually transmitted diseases testing; treatment for sexually transmitted diseases
- Vaginal and urinary tract infection checks and treatment.
- Pregnancy testing and options counseling.
- Referrals for abnormal pap smear followup.
- Contraceptive services including birth control pills, emergency oral contraception, intrauterine devices, Depo-Provera, Lunelle, Evra patch and Nuva Ring, diaphragms, cervical caps, spermicidal foam and condoms.
- Information and education upon request for topics of sex education, puberty, contraception, and sexually transmitted diseases
- All services are confidential.
Eligibility
All women of reproductive age who have not chosen a permanent method of fertility control. Limited services available for men.
Fees
Based on the ability to pay.

Purpose:
To protect children and adults from preventable communicable diseases.

Flu vaccine available in October and November at designated clinics; for additional information, please call our office.
The child's parent or legal guardian must accompany the child to sign the consent form. When bringing your children in for immunizations, please bring their immunization records and their medicaid, CHP+ or Rocky Mountain HMO insurance card if they are insured by these plans.
Fees
Call for current fees. Medicaid, CHP+ and Rocky Mountain HMO can be billed for immunizations.

Nurse-Family Partnership
Nurse-Family Partnership is a program which can help you and your child begin a healthy life.
Taking good care of yourself, preparing for childbirth, and then caring for your new baby will be challenging! We believe it will also be the most important and satisfying work you will ever do.
The goal of the Nurse-Family Partnership is to support you through your pregnancy and the first two years of parenting. Nurse-Partners are specially trained registered nurses willing to provide positive support and education for you to help you be the best parent you can be.
You are invited to enroll for a Nurse Partner if you are expecting your first child and have a limited income.
Families who are eligible for the Nurse-Family Partnership receive:
- Home visits during your pregnancy and after the birth of your baby.
- Answers to your questions about pregnancy, labor and delivery.
- Information about baby and early childcare.
- Support for family needs and links to community resources.
- Educational gifts and books.

Women, Infants, Children [WIC]
Purpose
To provide nutrition education and food for infants/children and pregnant and/or breastfeeding women.
Eligibility
Women who are pregnant or breastfeeding. Infants and children who are within income guidelines and have at least 1 nutritional risk factor qualify for WIC.
Fee
Must meet income guidelines, at or below 185% of poverty, which are similar to the reduced school lunch guidelines.
