Immunization Clinic

Purpose


The goal of the Immunization Clinic is to protect children and adults from vaccine-preventable communicable diseases.

Eligibility
We currently offer all required vaccines for children with Medicaid or who are un-insured, under-insured, or are American Indian or Alaskan Native*.
We also offer vaccines to adults who are uninsured. For those with insurance we offer the Tdap vaccine, tuberculosis skin testing, and influenza vaccine during season. Vaccines are also available at the following local clinics, where a variety of insurance is accepted (please call for details):

Immunizations


Routine immunizations for anyone under 18 years of age who qualifies:
  • Chicken Pox (varicella)
  • Diphtheria
  • Hemophilus influenza [HIB]
  • Hepatitis A
  • Hepatitis B
  • Measles
  • Meningitis
  • Mumps
  • Pertussis
  • Pneumococcal disease
  • Polio
  • Rubella
  • Tetanus
  • Human Papillomavirus (HPV) 
The child's parent or legal guardian must accompany the child to sign the consent form. When bringing your children in for immunizations, please bring their immunization records and their Medicaid, CHP+ or Rocky Mountain HMO insurance card if they are insured by these plans.

Routine immunizations for adults are:
  • Chicken Pox
  • Hepatitis A
  • Hepatitis B
  • Measles / Mumps / Rubella
  • Meningococcal Meningitis
  • Pneumonia
  • Tetanus / Diphtheria
  • Pertussis
*We do not currently offer the Shingles Vaccine.

Flu Vaccine


Flu vaccine is available in during flu season (check back for dates) at designated clinics; for additional information, please call our office.
